  • It is latral perforation caused by over instrumentation through a thin wall of the root.
  • Most common on the inside wall or concave, wall of curved canal (dangerous zone)
  • Such as distal wall of the mesial roots of lower 1st molars.
  • Commonly occur at 1.5 mm below bifurcation.
