1) Barriers, obstacles or problems which caused you to lack academic success all through the program?
2) What solutions and resolutions have you made to break down the barriers, obstacles or problems which had caused your poor academic presentation?
3) Pending your approval for being permitted to re-enter the University, what plans and strategies do you have for making the Satisfactory Academic Progress?
