Your client is a 38-year-old single parent who lives with her 19-year-old child in an inner-city, one bedroom apartment. She has a history of heavy alcohol (ETOH) use and occasional poly-substance drug use, although denies recent use of either. The client also has a history of peripheral vascular disease (PVD) and was recently hospitalized for amputation of the left great toe. Although the treatment was successful and restored pulses to the left leg, the amputation site has been slow to heal. There is a small 2 X 3 X 1 centimeter wound on the surgical site. The wound has reddened, macerated edges. The drainage is serosanguinous and thin with no odor. The client has orders for dressing changes twice weekly to the open wound at the surgical site. The dressing change technique is uncomplicated and easily managed. However, the client refuses to learn to change her own dressings. The physician orders a home visit to review dressing change techniques and teach patient how to manage dressing changes independently. During the home health visits, the nurse notes that the client always wears a heavy stocking on her right foot and keeps it tucked under while the dressing is being changed. The client refuses to let the nurse examine the right foot, saying, "the doctor just saw it, and its fine. It's only my left leg I have trouble with."
