In conjunction with aren't/caregiver, develop an individual action plan for improving the quality of life for self and children • Monitored, tracked & documented parent. /caregiver's progress in achieving goals • Reviewed Action plans with parent/caregiver at least every 30 days or as needed. • Updated and ensure logs and database are current and accurate. • Worked with local service delivery network to make service referrals. • Developed collaborative relationships with community-based organizations and agencies that provide support services and resources. • Reviewed and provides program and client-specific summary reports for management review. • Performed any other duties as assigned
