ASSIGNMENT:
Part A: Complete the Lifestyle Assessment Inventory. Record your scores for each of the lifestyle assessment sections, as well as the total score and answer the following questions.
1. What were you most surprised to learn about your lifestyle?
2. What disappointed you most about your lifestyle?
Part B: Based on your scores and answers to the above questions, create a personal wellness plan for this semester. This plan should include the following.
1. Which specific areas in the Lifestyle Assessment Inventory do you plan to pay attention to during the spring semester? (Pick at least three areas)
2. Why did you pick these specific areas?
3. What strategies do you plan to use to address these each of these areas? These strategies should be SMART strategies (i.e. Small, Measurable, Attainable, Realistic and Trackable).
4. How do you plan to track your progress in these areas throughout the semester?
