Case Scenario: Hamilton is a 32-year-old male who was involved in a car accident 10 months ago. His acquired brain injury has impacted on his memory, mobility and coordination. During Hamilton's last appointment with Dr Ahmandi, he admitted he was worried about his teeth and was experiencing pain when eating and drinking. Dr Ahmandi has created an Individual Action Plan to help develop the skills to maintain good oral health and to know the signs of possible gum disease and cavities. You are required to perform a skills assessment to help your client Hamilton Ng, improve his oral hygiene. You will need to discuss his personal strengths and goals for skill development and help him determine what resources are required. You will complete an Individual Support Plan (ISP) while speaking with Hamilton.
