You are the Executive Director of Autumn Gold, and your Board received a formal complaint from the daughter of a recently expired re Tim. In her complaint, she alleged the institution failed to provide proper care to her father, and as a result it contributed to his death. have decided to conduct a retrospective assessment of what transpired at Autumn Gold as it pertains to Tim. It is anticipated that this assessment will result in the identification of what actions or decisions may have unwittingly contributed to the death of Tim and then tangible steps that management will take to prevent similar incidents from occurring in the future. The Board would like you to prepare report of your findings coupled with a set of recommended steps for improvement. The report will become an attachment to a cover bus memorandum, which should be written to the attention of Ms. Harrison, Chairperson of the Autumn Gold Board of Trustees.
