Common faults found are:
- Combustible items left in protected routes
- Locked exits
- Fire doors wedged open
- Exits obstructed by stored goods
- Faulty fire doors, for example large gaps between double doors
- Insufficient staff training so that the staff are uncertain of what their actions should be in an evacuation
- Fire alarm and emergency lighting system not routinely tested by the staff
