Problem: Mrs. W is a 43-year-old client who has been experiencing bouts of headache for the past 2 months. Today, Mrs. W experienced a sudden onset of severe headache. She stated: "it is the worst headache that I have ever had and I feel really weak". On arrival in the ED, Mrs. W. is projectile vomiting, she continues to describe the pain as severe and states her eyes are blurry. She has a history of HTN and is taking Valsartan 80mg po daily. On assessment, Mrs. Is awake, alert and oriented to person, place and time. Her verbal responses are clear and easy to understand. She is able to follow commands, and move all extremities with equal strength bilaterally, PERRLA, she does have some nuchal rigidity VS: BP 170/96 HR 90 Resp 18 Temp 98.6 F CT reveals blood in the subarachnoid space, intracerebral clots and large clots surrounding an aneurysm. She is transferred to the Neuro ICU with the following orders: Bedrest with HOB at 45 degrees Prep for insertion of an arterial line Neuro assessments and BP q1hr and PRN Mannitol IV 0.5-1 g/kg over five to ten minutes Furosemide 40 IV prior to administration of mannitol Phenobarbital 100.
