Rewrite as a professional clinical note: I observed biological mother continuously "nod" off, while sitting in the waiting room attempting to complete two new patient packets. I observed this behavior/ actions for approximately 20-30 minutes. I attempted to assist Mother by asking her if she needed assistance completing the paperwork, since she had begun about 1 hour prior. Mother ignored my ask. She then asked her adult brother, "AJ" to give her his glasses, and stated that she was having difficulty seeing. Continuing to observe, she still demonstrated great difficulty completing the paperwork and continued to "nod" off. FD again checked in with mom about the status of completion of the paperwork. Mother brought up a medical assessment for patient and he was brought back with Uncle, while mom stayed in the waiting room with younger sibling to complete his paperwork. Mom then transitioned herself from a chair in the waiting area to sitting on the floor in a small play area off to the side of the reception area.
