Lynetta Discussion:

If the Chronic Care Connect Program is operating well but has fewer participants than expected, I would adjust our outreach and engagement strategies. According to Issel, Wells, and Williams (2021), effective program monitoring includes assessing whether recruitment strategies are reaching the intended audience and making necessary adjustments. This could involve strengthening community partnerships, leveraging social media, and enhancing provider referrals to increase awareness and participation. Also, gathering feedback from current participants can help identify barriers and refine recruitment efforts.

If program staff is struggling with required hours and incomplete training, I would implement a flexible scheduling approach, possibly redistributing workloads or offering condensed, high-priority training sessions. Training is essential for program success, and as Issel et al. (2021) highlight, ensuring staff competency before implementation is crucial to maintaining program integrity and achieving desired outcomes. Virtual or on-demand training options could also help staff complete their training in time.

If these issues are not addressed, the program's overall outcomes could be compromised. Process evaluation ensures that a program operates as planned, and failure to meet participation or staffing goals could result in reduced effectiveness and participant engagement (Issel et al., 2021). By proactively addressing these challenges, we can help ensure the Chronic Care Connect Program achieves its intended impact.

Amy Discussion:

If there were fewer participants than expected I would work with marketing to promote the program utilizing social media platforms and printing brochures. Social media is widely used now and is a great way to communicate a message to a large population. I would advertise on Facebook. Facebook is a platform where it can be shared among people to spread the news and help educate people. To market for people who are less on social media, providing brochures in community centers and within the hospital can help market the service.

I would reevaluate staffing schedules to ensure that there is sufficient coverage during office hours and provide online training for staff. Online training is cost effective and provides flexibility for employees to access at a convenient time.

These are two important components to the program as without participants, this will pose a financial challenge to the program. Staff members are crucial to this program as they are the ones providing the care to our participants so ensuring that they are fully trained and satisfied is just as important. Staff satisfaction contributes to productivity and a positive work culture ensures motivation leading to better outcomes for patient satisfaction.
