Assignment Task:
After learning about social determinants of health and gathering your county research, you will start working on preparing your presentation.
Prepare: Your final project should be prepared in a PowerPoint "type" format (PowerPoint, Prezi, etc.) with either voice-over or video presentation. Items in the presentation should include (but is not limited to):
Analysis of statistical data relating to county population (approx. 2-3 slides)
SDH strengths and needs in the county and how that relates to Healthy People 2030 goals (at least 1 slide for each)
Available county resources to address the SDH needs and ways to assist clients to accessing those resources (approx. 2-3 slides)
Incorporation of your patient profile
Presentation: Your final SDH presentation will be posted in the discussion below. Your presentation should be creative and include appropriate pictures or graphics. The presentation should be a maximum of 12-15 slides and 10-15 mins worth of presentation (video/voice-over) time. Need Assignment Help?
