African american male presents with this rash


Problem: A 59-year-old African American male presents with this rash observed below. The rash first appeared about 7 years ago. He has seen various providers since that time and was told that he has eczema. He said that this was based on a biopsy done 5 years ago. He denies a history of seasonal allergies, asthma, or eczema as a child. Treatment with topical corticosteroids provides some temporary improvement, however the rash has never resolved, and in fact, seems to slowly have expanded and progressed to other parts of his body. You perform a KOH which is negative. What would be the best next step in the management of this patient?
