Problem: Address the unique needs of individuals and families to assist them toward self-determination of their health care needs. (SLO1) Being able to have self-determination goes beyond meeting the immediate needs of a person when it comes to healthcare needs. It involves encouraging individuals to take an active role in managing their health long-term. As the nurse I would educate individuals about preventive care and healthy lifestyle choices, encourage self-management of chronic conditions through education and support programs and advocate for patients' rights and ensure they have the information and resources to make informed decisions about their care. A patient-centered approach is a great way to actively involve patients and their families in the decision-making process. This can be achieved through shared decision-making models where healthcare providers inform patients about treatment options and collaborate with them to choose the best course of action (Smith & Johnson, 2020). Another great way to assist would be to ensure easy and affordable access to healthcare resources including healthcare facilities, medications, and supportive services.
