The pressure within the middle ear is maintained equivalent to the external ear (as a result to exterior as well) due to a communicating duct between the middle ear and the pharynx termed as the auditory tube, or Eustachian tube. Whenever someone goes down a mountain, the air pressure on the middle ear rises and it is essential to do some exercises such as fake swallowing to force the opening of pharyngeal orifice of the auditory tube to make equal the pressure again.
